A QR Code will be available to scan at the Hall for anyone who has a smart phone and the NHS COVID-19 app. As a print at home alternative to the NHS COVID-19 app please download the form linked below.

If you do not have a smartphone or the NHS COVID-19 app, please fill out this form prior to arrival at the Hall, and have the completed form ready to give to a steward on arrival.

The information supplied will be held by the Hall for 21 days. After 21 days, this information will be securely disposed of.

Advice for Downloading & Using NHS Covid-19 App

The quickest way to provide your ‘Track and Trace’ details at The Bridgewater Hall is via the NHS COVID-19 app. The NHS COVID-19 app is completely anonymous and is the fastest way to see if you’re at risk from coronavirus. The app has a number of tools to protect you, including venue check-in.

Venue Check-in
When you check into venues using the app and official NHS QR code posters, the data is held on your phone. If it’s identified that people who were there on the same day have since tested positive for coronavirus, you will get an alert with advice. The name of the venue will not be revealed. Alerts will generally only be sent for outbreaks that happened in the last 7 days.
